This form is required for all applicants and AFFC providers. AFFC applicants and providers will put in writing their plan should there be need to relocate in the event of a disaster. The purpose is to insure AFFC providers and recipients can be located in the event of an evacuation.
This form is completed by the AFFC provider applicant as part of the application process for an initial license.
